In order for your child to be considered for this scholarship, you MUST submit the following documents with this application:

  • Completed application (click here for link)
  • Final report card for the 6th and 7th grades
  • Most recent report card for the 8th grade
  • PSSA scores for the 6th and 7th grades
  • High School Placement Test Scores
  • Written statement from applicatant
  • Copy of acceptance letter from the prospective high school
  • Copy of the financial aid award from the prospective high school
  • Two (2) letters of recommendation from teachers on ADVIS forms provided with application
  • Copy of parents’ most recent 1040 and W2 tax forms
  • Copy of Parent Financial Statement (PFS)
